Getting work permit 16?

Im 16. This year I've been so preoccupied with track and my academics that I forgot to fill a work permit form. Now, its near the end of school and I only have 3 days to turn it in to the 'jobs and career center' at my highschool. Worse still, I don't know how to apply to raleys or any big name markets. I also have little jobs skills, though i have done community service in a small thrift store.
